Durante un'escursione in Africa, Sir Edward Markham rimane vittima di una stregoneria, assumendo un aspetto mostruoso che costringe il fratello Julian, una volta tornati in Inghilterra, a tenerlo segregato in un tetro castello. Ritenuto erroneamente morto, Edward viene sepolto: salvato da un medico che trafuga cadaveri per i suoi esperimenti egli è animato dal solo desiderio di vendicarsi di coloro che ritiene responsabili della sua sepoltura. Celando il volto mostruoso dietro una maschera rossa, Edward inizia così una serie di feroci delitti; ma, affrontato dal fratello Julian viene da questi ucciso. Prima di morire, egli riesce però a mordere il fratello, trasmettendogli il proprio maleficio: col volto orribilmente deturpato, Julian sarà costretto a trascorrere il resto dei suoi giorni segregato in una soffitta.
