L'ex commando americano John Rambo si è trasferito in un convento buddista dove spera di trovare la pace interiore. Qui viene raggiunto dalla notizie che il suo ex-colonnello Sam Trautman è stato catturato, nel corso di una missione di spionaggio, dalla truppe sovietiche in Afghanistan.
