John Merrick è stato seriamente sfigurato alla nascita da una malattia rara. Il suo viso vistosamente deturpato mostra i lineamenti di un elefante. Intrappolato in una fiera di fenomeni da baraccone Merrick patisce il tormento di essere un reietto della società finché un giovane chirurgo chiamato Dr. Frederick Treves( Sir Anthony Hopkins) lo libera.
